Field Nursing

Purpose:

Field Nursing staff provide case management services throughout Humboldt County. The goal is to meet the health needs of families and individuals and promote positive health behavior and improved health outcomes. We work with families/individuals to identify needs via nursing assessments and screening tools. We assist clients in accessing medical care and community-based resources. Field nursing promotes self-advocacy while providing education and support for building resiliency and creating a safe, stable and nurturing environment for families and individuals, including:

  • Individuals with high risk prenatal (pregnancy)
  • Individuals with high-risk postpartum (after delivery)
  • High risk infants
  • Child(ren) in the home age 0 to 5 years old with chronic medical condition(s) or unmet needs
  • Parents, caregivers or children who have significant risk factors or medical needs and/or need support but do not meet requirements for other programs.
  • Willow Creek and Garberville offices serve families and individuals with medical needs and/or those needing assistance linking to resources.

The Field Nursing team is comprised of Public Health Nurses and Community Health Outreach Workers.
