If you are currently opting out of health insurance through the County of Humboldt, annual verification of other health insurance coverage will be required in order to continue to receive the Opt Out differential payment for calendar year 2020. Please complete and return forms with necessary additional documentation to Human Resources/Risk Management. The documents can be emailed to email@example.com, faxed to 707-268-2546, inter-office County mail to HR/Risk Management, or USPS to Risk Management, 825 5th Street, Room 100, Eureka CA 95501.
The deadline for submitting Verification of Coverage is at 5:00 p.m. on Friday, November 22, 2019. Those individuals who do not provide the completed Verification Form and proof of other health insurance coverage, if applicable, will no longer continue to receive the County Opt Out differential payment effective January 1, 2020.
Please Note: If you enrolled in the Opt Out on or after June 1, 2019, verification of health insurance coverage will not be required until next calendar year.
